Patient Forms

We want your first experience with us to be a positive one. Your initial visit will take around 90 minutes, so to save some of your time, please download and fill out our new patient form online and bring it with you to your appointment, so that we can get to know you better.

During your first appointment, Dr. Zusko and our team will review your completed forms and perform the necessary diagnostic testing to allow Dr. Zusko to properly recommend a personalized treatment plan to fit your needs. The initial consultation appointment is $150.

We require 48 hours cancellation notice for any appointment. Any missed appointment or late notice cancellation will result in the patient being charged the missed appointment fee. If you miss your first consultation and wish to reschedule, you will be charged a total of $300 in advance to cover the cost of your missed appointment as well as your new one before a new consultation appointment will be made for you.
